Medical Form

‍All persons both youth and staff must submit an application online to registrar@seascoutcup.org. Download the application to your computer, complete the fill in form and then attach it to an e mail and transmit it to the registrar or mail it to the registrar.

After your application is submitted you will be advised by e-mail if you are accepted. Once accepted you have 3 weeks to submit payment and medical form.
